If the pet goes missing, I agree to contact FOPAS within 24 hours.

If the pet is picked up by animal control, I agree to reclaim the pet or contact FOPAS to reclaim.

If FOPAS should be made aware that a pet adopted from us is at a shelter or other holding facility ("other holding facility" to include but not limited to a veterinarian's office) for more than 48 hours and I have not notified FOPAS that the pet is missing, FOPAS has the right but not the obligation to reclaim the pet as its own.

I am aware of the adoption fees for FOPAS pets. All animals are spayed/neutered, up to date on age appropriate vaccinations, and microchipped.

By submitting this application, I certify that the information I have given in this application is true and that any misrepresentation of facts may result in my losing the privilege of adopting a pet from FOPAS, now and in the future. I understand that FOPAS has the right to deny my request to adopt an animal and I authorize investigation of all statements in this application.
