Rabbit Pre-Adoption Application
These should be people who have observed you with your previous pet(s) or can speak to your character if you have never owned a pet before Only one of these references may be a relative. If for some reason we are unable to reach these people we may contact you for more information or another reference *
Adoption fee for rabbits is $100. All rabbits are spayed/neutered, microchipped, and examined by a vet. There is a two week trial period to ensure it is a good fit. If it is not working out, the rabbit will be returned and the adopter will receive half their money back. If the trial goes well and the adoption is finalized, we will transfer the rabbit's microchip into the adopter's name and provide their medical paperwork via mail. If at any point throughout the rabbit's life the adopter is no longer able to care for the rabbit, it will be returned to FOPAS.
If FOPAS should be made aware that an animal adopted from us is at a shelter or other holding facility ("other holding facility" to include but not limited to a veterinarian's office) for more than 48 hours and you have not notified FOPAS that the pet is missing, FOPAS has the right but not the obligation to reclaim the dog as its own.* Your signature below is confirmation that you understand this and agree to these terms.
By submitting this application I certify that the information I have given is true and that I recognize that any misrepresentation of facts may result in my losing the privilege of adopting a pet and nullification of any contract in the future. I understand that the Friends of Parkville Animal Shelter have the right to deny my request to adopt an animal and I authorize investigation of all statements in this application I understand that this application is the property of Friends of Parkville Animal Shelter. I understand that an in person home visit is required to adopt the rabbit. Your signature below confirms you understand and agree to these terms.
